• This free prize draw is open to all those who leave feedback and supply their contact details, excluding employees of Willows Veterinary Centre & Referral Service, their agents, or anyone professionally connected with the prize draw.
• Only one entry per household per visit will be accepted.
• Entries from third parties, bulk entries or entries via agencies will not be accepted.
• No purchase is necessary.
• All entries can be submitted by completing the feedback form (available from reception) and submitted either by posting in the postbox in the reception area or posting back to us using the reply-paid envelope or by completing the online feedback form.
• The first completed feedback form drawn from all entries receieved for that quarter (quarters run from 01 January-March 31; 01 April-30 June; 01 July-30 September; 01 October-31 December) will win a Marks and Spencer Gift Voucher for £100
• Prize is offered as stated and are non-exchangeable or transferable. No cash alternatives will be provided.
• Once notified by us, the winner will be contacted to arrange delivery of the vouchers.
• We reserve the right to pick an alternative winner if the original winner does not contact us within 14 working days of being informed that they have won.
• We reserve the right to cancel or suspend any prize at any point, without liability to the prize-givers or winners.
• Our decision is final on all matters and we will not enter into any further correspondence.
• Entry into the prize draw constitutes acceptance of the rules.
